2. Name, address, designation, and nationality of the signature authorities of General Power of Attorney.

3. 10 Numbers of Prints or photographs of designs with several sides which may clearly reveal the clear shape, size, and configuration.

4. Written proof of priority, if any.

5. Duly signed power of attorney by Proprietor, partner, one of the directors of the company.

6. Copy of MOU & AOU or copy of partnership deed.
